About this resource
This program assists older adults and individuals with disabilities in accessing services by evaluating their needs and coordinating with service providers. It also provides additional support for Medicaid recipients who may otherwise require placement in a nursing facility.

- Individuals aged 60 and above, adults with disabilities starting from age 18, and family caregivers residing in Kittitas, Yakima, Benton, Franklin, Walla Walla, Columbia, Garfield, or Asotin counties.
