Nurses, Behavioral Health Specialists, and Case Managers offer outreach services on the streets to locate and assist unhoused individuals who require support and a way off the streets.
Intake Process: Contact us to inform about the location of the person(s).
Cost: Free of charge.
Who can use this
- Assistance is available for those in need, with families and individuals eligible for support up to three times annually.
