Humphreys Diabetes Center - Boise

St. Lukes Humphreys Diabetes Center (SLHDC) is a community initiative acknowledged by the American Diabetes Association. It provides a wide range of services, including basic self-management classes for individuals of all ages, personalized consultations, an outpatient insulin program, an insulin pump program, a gestational diabetes program, Hemoglobin A1c testing, diabetes prevention classes, and behavioral health services. Individual services are also available in Fruitland, while classes and consultations are offered at the Nampa and Meridian locations. The self-management classes cover all facets of daily diabetes care, such as nutrition, blood sugar monitoring, exercise, and coping strategies. The outpatient insulin program allows patients to begin treatment without hospitalization, and Hemoglobin A1c tests can be scheduled at the center with a physician's order. Training for insulin pumps and continuous glucose monitoring is also provided. Most insurance plans, including Medicare, cover diabetes education benefits, but a physician's referral is necessary for reimbursement. Financial assistance is available based on need, as determined by St. Lukes patient financial services. Services are offered in English and Spanish, with translators available for other languages.
