This service is dedicated to assisting patients aged 65 and older in attaining optimal health and autonomy by delivering specialized medical services directly to their homes. It includes a range of skilled healthcare services such as nursing, physical therapy, occupational therapy, speech therapy, social services, dietary consultation, and traditional in-home skilled services that cover medication management, wound care, and education on managing various conditions like diabetes, congestive heart failure, chronic obstructive pulmonary disease, and urinary catheter care. Additionally, it features four specialized programs: Fall Prevention (Safe Strides Program), Orthopedics, Low Vision, and Neuro Rehabilitation. The service accepts Medicare, Medicaid, and other health insurance plans. Available languages include English.
