Collaboration Center Foundation

The program offers Pathways Navigation, Community Referral, and Case Management services aimed at assisting individuals, their families, caregivers, and providers in understanding the resources and funding options available for those with intellectual, developmental, and physical disabilities (I/DD/PD). Current services include Pathways Family Inclusion Programs (complimentary support groups and social activities), Neuropsychological Testing, Mental Health Services, ABA Therapy, Occupational Therapy, Speech Therapy, Physical Therapy, IEP Advocacy, Vocational Training Programs, and Social Skills Groups/Activities. Services are available in Spanish and English.
