About this resource
Delivers a wide array of integrated medical and wellness services that encompass adult and pediatric primary care, management of chronic diseases, nutrition and wellness support, case management, a pharmacy, STI testing and treatment, behavioral health, addiction medicine, care coordination, and additional support within a cohesive system. Available languages include Spanish and English.

Fees: Services are offered on a sliding scale, and Medicaid fee-for-service (FFS) is accepted.
Who can use this
- Patients are welcomed regardless of their insurance coverage. Those with HMO plans should contact their insurance provider to designate HOPES as their provider prior to making an appointment.
