This program offers Care Management Services for adults on Medicaid who are dealing with serious mental illnesses (SMI), HIV/AIDS, or two chronic health conditions, as well as case management services for children's health homes.
Intake Procedure: Individuals can either walk in or make a phone call to access services.
Fees: There are no associated costs.
Who can use this
- Individuals aged 18 and above who are Medicaid recipients and possess a Record of Qualifying Diagnosis. Service areas include Allegany and Cattaraugus Counties.
