About this resource

This program offers Care Management services for children on Medicaid who are diagnosed with serious emotional disturbances (SED), HIV/AIDS, two chronic health conditions, or complex trauma.

Intake Process: You can either walk in or make a phone call.

Who can use this

  • Children aged 12 and under who are Medicaid recipients and have a diagnosis of SED, HIV/AIDS, two chronic health conditions, or complex trauma are eligible. A qualifying diagnosis record is required. Service Area: Cattaraugus and Allegany County.
