About this resource
This program provides permanent supportive housing along with case management and rental subsidies. Participants collaborate with case management services to address obstacles and work towards their goals. The primary aim is to help chronically homeless individuals and families secure stable housing and attain financial self-sufficiency. Referrals can be made from agencies serving homeless populations. We gather information on previous living conditions and create a tailored plan that outlines the actions the case manager will take, as well as the steps clients need to follow to achieve their objectives.
