This initiative provides Health Homes and Ryan White care management services to individuals affected by various chronic conditions (such as HIV, mental health issues, asthma, diabetes, and Hepatitis C) across Broome, Chemung, Chenango, Cortland, Delaware, Otsego, Tioga, and Tompkins Counties.

Cost: No charge.
Who can use this
- Individuals with HIV/AIDS, Serious Mental Illness (SMI), or two additional chronic conditions (like diabetes, COPD, CHF, hypertension, asthma, or depression).
