About this resource
This initiative supports Medicaid-eligible individuals with TBI in moving out of nursing homes into the community or in obtaining services that help them avoid nursing home placement. We assist individuals in navigating the eligibility process, choosing a service coordination agency, and accessing various benefit programs and services available in the community.
