About this resource
This initiative aids Medicaid-eligible individuals with TBI in moving out of nursing homes into the community or in obtaining services to prevent nursing home placement. It guides individuals through the eligibility process, assists in choosing a service coordination agency, and helps them access various benefit programs and community services.

- Available in Allegany, Broome, Cayuga, Chemung, Chenango, Cortland, Delaware, Otsego, Schuyler, Steuben, Tioga, and Tompkins Counties.
