This initiative provides assistance to children dealing with significant emotional disturbances, developmental disabilities, or medical fragility. It includes Child & Family Treatment Support Services, Home & Community Based Services, and Health Home Care Management.

Who can use this
- Children and young adults from birth to 21 years who have faced trauma or suffer from serious health or behavioral challenges and possess Medicaid or qualify for it; eligibility is assessed individually and may differ based on the type of service.
