This program provides care coordination for adults facing mental health challenges who do not qualify for Medicaid. Its goal is to minimize repeated visits to hospital emergency rooms in Cattaraugus County.
Intake Process: Referrals can be made by others, self-referrals, phone calls, or in-person visits.

Who can use this
- Adults aged 18 and over with serious mental health conditions who lack Medicaid, but might have Medicare or other insurance. Service Area: Cattaraugus County.
