This program offers Care Management services for children who are enrolled in Medicaid and have a diagnosis of severe emotional disturbance (SED), HIV/AIDS, two chronic health conditions, or have experienced complex trauma.
Intake Process: You can either walk in or make a phone call to start.
Cost: There are no fees associated with these services.
Who can use this
- Children aged 21 and under who are on Medicaid and have a diagnosis of SED, HIV/AIDS, two chronic health conditions, or severe trauma, with a documented qualifying diagnosis. Available in Chautauqua County.
