This program offers support to families coping with illness and home care, including psycho-social counseling for long-term planning and decision-making, assistance with nursing home and adult home placements, as well as community resource planning and referrals.
Intake Procedure: Clients are encouraged to call the agency for further information.
Fees: Services are offered on a sliding scale, and may be covered by Medicaid, Medicare, or insurance; some services are provided at no cost.
