About this resource
Supported Employment: The program provides services such as pre-employment assistance, job placement, ongoing support after employment, case management, crisis intervention, and job coaching. Transportation options may be available based on job placement, but are not guaranteed. Vocational Assessment Services: This includes vocational evaluations, career counseling, pre-employment assessments, and connections to other resources. Driver's Rehabilitation: Services consist of training, clinical evaluations, and assessments for vehicle modifications, specifically for individuals with significant limitations.

Fees: Services are funded through ACCES-VR or OPWDD.
Who can use this
- Supported Employment: Eligibility is through ACCES-VR/CBVH/OMH for individuals aged 21 and older. Vocational Assessment Services: Referrals can come from ACCES-VR/CBVH/DSS/Medicaid/Physicians/Schools. Driver's Rehabilitation: Available for those aged 16 and above with ACCES-VR eligibility. Service Area: Chautauqua County.
