About this resource
This program provides outpatient medical services for individuals and families, which encompass primary and preventive care, pediatrics, family medicine, dental services, obstetric and gynecologic care, geriatrics, podiatry, nutrition, immunizations, speech and hearing screenings, and Medication Assisted Treatment (MAT) for opioid addiction. Additionally, it includes short-term mental health and addiction services, as well as optometry services such as vision and glaucoma testing, eyeglass prescriptions, and diabetic eye screenings.

Fees: Accepts Medicaid and offers a sliding scale based on income, with discounts available up to 75%. For more details, please call.
