This program includes a family support initiative featuring a support group for parents, care and case management services, individual residential options through supportive apartments, and employment assistance aimed at competitive job placements. Additionally, it offers a day habilitation program.

Who can use this
- Participants must be eligible with OPWDD, enrolled in Medicaid, and receiving care coordination through Prime Care or Person Centered services.
