About this resource
This program offers assistance to individuals with developmental disabilities, including evaluating service needs, creating and coordinating service plans, making referrals, providing follow-up, monitoring progress, and advocating for clients. The agency maintains ongoing communication with the individual, their primary caregiver, and service providers to facilitate the planning and acquisition of necessary services and supports.

Who can use this
- Individuals aged 18 and older diagnosed with developmental disabilities who require case management services are eligible. There are no age limits for those with Medicaid, but they must comply with Medicaid eligibility and residency requirements.
