Community Mainstreaming Associates

This initiative provides residents of Nassau and Suffolk Counties who have intellectual and/or developmental disabilities, along with their families, the opportunity to choose services and providers that best meet their requirements. It includes self-directed community habilitation, respite services, and SEMPT (supported employment). With the help of a Fiscal Intermediary and Support Broker, individuals and their families are guided in implementing HCBS (Home & Community Based) waiver services, creating a habilitation plan, assessing their needs, and managing their allocated Medicaid funds.
