This program offers case management services to support children facing significant mental health challenges or chronic health issues, helping them maintain their health and avoid emergency care or hospitalization.

Who can use this
- Eligible children are those aged birth to 21 years who have: 1) Active Medicaid 2) A diagnosis of serious emotional disturbance, HIV/AIDS, or other mental health conditions.
