About this resource
This program provides case management support to adults facing chronic health issues, serious mental illnesses, or HIV/AIDS, aimed at helping them maintain their health and avoid inpatient care, hospital visits, and emergency room use.

Who can use this
- To qualify, individuals must: 1) Have active Medicaid 2) Suffer from two or more chronic health conditions, OR have HIV/AIDS, OR have a serious mental illness.
