About this resource
The Case Management program aims to assist children and adolescents who are severely and persistently mentally ill or seriously emotionally disturbed. Operating around the clock, this initiative delivers proactive outreach and support to those who frequently utilize emergency and inpatient services or disengage from care. A dedicated team focuses on children at risk of hospitalization or needing placement. The program is equipped to offer wraparound services and requires a referral from an outpatient treatment program for access.
