About this resource
The Case Management program aims to assist adults who are severely and persistently mentally ill or seriously emotionally disturbed. This program operates 24 hours a day, offering proactive outreach and support to those who tend to overuse emergency room and inpatient services or who may withdraw from treatment. Access to the program is through referral.
