About this resource
The âItâs About Youâ initiative is dedicated to enhancing infant and maternal health outcomes by adopting a life course approach that targets clients at different stages, including preconception, prenatal/postpartum, and interconception. The campaign seeks to empower both women and men to take charge of their health through the provision of educational resources, access to community support services, and advocacy efforts. Key components of the program include the involvement of Community Health Workers, home visits, risk assessments, personalized service plans, referrals, preventive screenings, and health coordination to achieve these goals.
