About this resource
The program assists older adults aged 60 and over who are ineligible for Medicaid, aiming to prevent costly and premature admissions to nursing facilities for them and their families. It provides case management and non-medical supportive services, which may include aide services for light housekeeping, personal care, and respite care. Additionally, a consumer-directed model is available, allowing care recipients to hire someone they know for assistance.

- Elderly individuals with chronic physical and/or mental impairments residing in Saratoga County who do not qualify for Medicaid.
