Who can use this
- To qualify, individuals must be Medicaid recipients, have a Medicaid insurance plan, or be eligible for Medicaid, and must have a diagnosis of two or more chronic physical conditions, HIV/AIDS, or a serious, persistent mental illness, along with significant behavioral, medical, or social risk factors.
