About this resource
This initiative aids youth and their caregivers in obtaining essential services aimed at improving the health of young people. As part of the Health Home Care Management framework through CHHUNY, LLC, a care manager is responsible for coordinating, advocating, and educating to assist children and youth who are enrolled in Medicaid and meet specific health criteria.

Who can use this
- Eligible youth are those from birth to age 20 who are enrolled in Medicaid, have two or more chronic health conditions or a single qualifying condition such as HIV/AIDS, Serious Emotional Disturbance, Complex Trauma, or Sickle Cell Disease, and live in Chemung, Schuyler, Steuben, or Yates counties.
