Lakeview Health Services

The program provides support for adults diagnosed with mental health issues through community residences, supportive apartments, and single room occupancy (SRO) living. It includes psychiatric rehabilitation services, counseling, and assistance in developing essential life skills such as nutrition, financial management, healthcare, time management, coping strategies, and community engagement. Additionally, it offers long-term and transitional programs, case management, and housing-related services in the individual's own unlicensed home. Locations include Horizon Scattered Site Treatment Apartments, West End Heights Single Site Treatment Apartments, Markham House Community Residence, and a 38-bed Single Room Occupancy facility. For intake, applicants must go through the Single Point of Entry (SPOE), which involves an interview and site visits. Fees may be covered by SSI/SSA, Medicaid, public assistance, or private pay.
