Hudson Valley Community Services - Ulster

This initiative provides assistance in obtaining medical and dental services, specialty care, housing support, social security disability income, supplemental social security income, Medicaid, AIDS drug assistance programs, emergency financial aid, mental health services, and legal support, which includes health care proxies, living wills, and permanency planning. HVCS also assists clients who lack Medicaid or private insurance through grant funding. Each client requiring case management is paired with a dedicated case manager, who provides services through both office and home visits.
