Intake Procedure: Contact to obtain information and arrange an appointment.
Fees: Accepts several insurance plans for comprehensive services, including OpenCard/CAWEM Plus, CareOregon Dental, and ODS Dental-Unassigned. Sliding scale fees are available based on household income and size, with a minimum payment of $40 due at the time of the visit, which will be credited to the bill. Uninsured patients can receive urgent care services.
