About this resource
This program delivers confidential case management services to those living with HIV and AIDS. It offers financial support for HIV medications, transportation to medical appointments, insurance premiums, transitional housing, rent assistance, dental care, disease management education, and referrals to other agencies for basic needs, employment, and substance abuse treatment.
Intake Procedure: Contact us to arrange an intake appointment.
