About this resource
This program delivers private case management and independent living services for those living with HIV. It offers financial support for HIV medications, transportation to medical appointments, insurance premium payments, transitional housing, rent assistance, dental care, disease management education, and referrals to other agencies for basic needs, employment, and substance abuse treatment.
Intake Procedure: Schedule an appointment by phone.
