About this resource
This program supports families who encounter obstacles in obtaining health care. Community health workers and certified application counselors assist with health insurance applications, coordinate care, provide comprehensive support, and connect families to local resources.
Intake Procedure: For more information, please call; referrals are accepted from therapists, parents, teachers, primary care physicians, psychiatrists, and community organizations.
Fees: There are no charges; may bill Oregon Health Plan (Medicaid).
