About this resource
This initiative delivers confidential case management and independent living support for those living with HIV. It includes financial aid for HIV medications, transportation to medical appointments, insurance premium payments, transitional housing, rent assistance, dental care, education on disease management, and referrals to other agencies for basic necessities, employment, and substance abuse treatment.
Intake Procedure: Schedule an appointment via phone.
Fees: No charges apply.
