About this resource
This initiative delivers confidential case management and independent living support to individuals diagnosed with HIV. It includes financial aid for HIV medications, transportation to medical appointments, insurance premium support, transitional housing, rent assistance, dental services, education on disease management, and referrals to other organizations for basic needs, employment opportunities, and substance abuse treatment.
Intake Process: Schedule an appointment via phone.
Cost: No fees involved.
