About this resource
This program delivers confidential case management and independent living support to those diagnosed with HIV. It offers financial assistance for HIV medications, transportation to medical appointments, case management visits, insurance premiums, transitional housing, rent support, dental services, disease management education, and referrals to other agencies for basic necessities, employment opportunities, and substance abuse treatment.
To begin the intake process, individuals should call to schedule an appointment.
There are no associated fees.
