About this resource
This program provides care coordination and comprehensive treatment planning for young individuals. Services may encompass community-based treatment, day treatment, treatment foster care, respite care, psychiatric residential treatment, or other tailored services.
Intake Procedure: You can locate referral packets on the website by selecting "Looking for Forms?" on the left side of the page. Please fax the completed referral packet, directing it to "Wraparound Referral."
Travel/Location: Situated next to the Hawthorn Farm MAX stop.
Fees: No charges apply.
Who can use this
- Youth aged 0-19 must fulfill specific criteria: -Experiencing significant impairment in multiple areas (home, school, social relationships) -Involvement with multiple agencies (Special Education, Child Welfare, Juvenile Justice) -Previous outpatient treatment has been attempted without meeting the youth's needs.
