Their role is to help these individuals prevent hospitalization and reach their personal goals by connecting them with vital community resources, such as treatment, entitlements, housing, education, and vocational training.

Who can use this
- Individuals must have a documented history and current diagnosis of serious mental illness, substance abuse, or two or more chronic physical conditions, be eligible for Medicaid, and reside in Suffolk County.
