About this resource
Health-related social needs (HRSN) refer to fundamental requirements that influence an individual's health. These benefits are designed to aid qualifying members with essential needs such as assistance with rent and utility payments, tenant support, and storage costs. HRSN housing services can be accessed by members only once throughout their lives.

Fees: There are no associated fees.
Who can use this
- Eligible Advanced Health members must be either 65 years or older, have children aged 0-5, be pregnant or have given birth in the last year, or have a qualifying health condition, along with a household income at or below 30% of the area's average, a lease agreement, and a need for housing support.
Documents you may need
- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
