About this resource
Health-related social needs (HRSN) encompass fundamental requirements that influence an individual's health. These benefits are designed to support eligible members with essential needs, such as assistance with rent and utility payments, tenant support, and storage fees. Importantly, members can access HRSN housing services only once throughout their lifetime.
Intake Procedure: For more details, individuals can call, email, fax, or visit the website. CareOregon clients have the option to ask questions via an online portal, with typical response times ranging from 3 to 10 days. Visit: www.careoregon.org/members/member-portal.
Fees: There are no fees associated with these services.
Who can use this
- CareOregon members must meet the following criteria: - Be 65 years or older, have children aged 0-5, be pregnant or have given birth in the last 12 months, or have a qualifying health condition - Have a household income at or below 30% of the average annual income in their area - Require assistance to remain in housing - Hold a lease or written rental agreement - Lack resources to avert homelessness.

- -Medicaid ID number, Rental/lease agreement, or utility bill-Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
