Health-related social needs (HRSN) refer to fundamental necessities that impact an individual's health. These benefits are available to eligible members, offering support for essentials like rent and utility payments, tenant assistance, and storage costs. Each member can access HRSN housing services only once throughout their lifetime.

Fees: There are no fees associated.
Who can use this
- Members of Columbia Pacific CCO must meet these criteria: -Aged 65 or older, children aged 0-5, pregnant or recently gave birth, or have a qualifying health condition -Household income at or below 30% of the area's median income -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avert homelessness.
Documents you may need
- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
