Health-related social needs (HRSN) encompass essential needs influencing an individual's health. The program offers assistance to eligible members for fundamental needs such as help with rent and utility payments, tenant support, and storage costs. Each member is limited to receiving HRSN housing services a single time throughout their life.

Who can use this

  • Members of Columbia Pacific CCO must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth within the last year, or anyone with a qualifying health condition -Household income at or below 30% of the average annual income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avert homelessness.

Documents you may need

  • -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
