About this resource

This service provides replacement EBT cards for individuals who have lost or damaged their cards. Clients can either call or visit a DSHS office to ask for a replacement to be sent to their address. In certain cases, such as when a client lacks a permanent address, they may receive a replacement directly at the office.

Intake Procedure: To obtain a new card, clients can either call or go to a community service office. Those who qualify for an in-office replacement include individuals with a general delivery address, those living in a domestic violence shelter, cases where the CSO made an error resulting in the card being sent to the wrong address, clients whose cards were lost due to a natural disaster, instances where JP Morgan Chase returned the card via mail, or when the case is closed but benefits are still active.

Fees: There are no associated fees.
