About this resource
This initiative provides private case management and independent living support tailored for individuals diagnosed with HIV. It includes financial aid for HIV medications, transportation to medical appointments, case management sessions, insurance premiums, transitional housing, rent assistance, dental services, disease management education, and referrals to other organizations for basic necessities, job assistance, and substance abuse treatment.
Intake Procedure: Schedule an appointment by calling.
Fees: There are no charges.
