About this resource

Health-related social needs (HRSN) refer to essential requirements that influence an individual's health. The program offers assistance to eligible members for fundamental needs such as help with rent and utility payments, tenant support, and storage costs. Each member is entitled to access HRSN housing services only once throughout their lifetime.

Intake Procedure: For more information, you can call, email, fax, or visit the website; all necessary documents must be submitted for screening.

Fees: There are no fees involved.

Who can use this

  • To qualify, Eastern Oregon CCO members must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth in the past year, or have a qualifying health condition -Household income at or below 30% of the average income in their area -Require assistance to maintain housing -Possess a lease or rental agreement -Lack resources to avoid homelessness.

Documents you may need

  • -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
