About this resource

Health-related social needs (HRSN) encompass fundamental requirements that impact an individual's health. These benefits assist eligible members with essential needs such as rent and utility payment aid, tenant support, and storage fees. HRSN housing services can only be accessed once in a lifetime by members.

Intake Procedure: For more information, you can call, email, fax, or visit the website; all necessary documents must be submitted for screening.

Fees: There are no fees involved.

Who can use this

  • Members of the Eastern Oregon CCO must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth within the last 12 months, or anyone with a qualifying health condition -Household income at or below 30% of the average annual income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avert homelessness.

Documents you may need

  • -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
