About this resource
Health-related social needs (HRSN) encompass fundamental requirements that influence an individual's health. The program provides assistance to eligible members for essential needs such as rent and utility payments, tenant support, and storage costs. Each member is entitled to access HRSN housing services only once throughout their lifetime.
Intake Procedure: For more information, you can call, email, fax, or visit the website; all necessary documents must be submitted for screening.
Fees: There are no fees associated.
Who can use this
- HealthShare members must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth in the last year, or anyone with a qualifying health condition -Household income at or below 30% of the average yearly income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement; those without may complete a "verification of landlord-tenant relationship" form with their landlord to apply -Lack resources to avert homelessness.
Documents you may need
- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
