Health-related social needs (HRSN) refer to essential requirements that impact an individual's health. These benefits provide assistance to eligible members with fundamental needs, including help with rent and utility payments, tenant support, and storage costs.

Who can use this
- Members of IHN CCO must meet the following criteria: - Be aged 65 or older, children aged 0-5, pregnant or have given birth in the last 12 months, or have a qualifying health condition - Have a household income at or below 30% of the average yearly income in their area - Require assistance to maintain housing - Have a lease or written rental agreement - Lack resources to avert homelessness - Services can be accessed only once per lifetime.

- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
